If you’re new to Blue Shield Medicare Advantage Prescription Drug Plan, you can sign up for an optional supplemental dental HMO or PPO plan right on the application. Be sure to include the name of your dentist, if signing up for the Optional Supplemental Dental HMO plan.

If you’re already a Blue Shield Medicare Advantage-Prescription Drug Plan member, you have three convenient ways to enroll in one of our optional supplemental dental plans:

Medicare Advantage Prescription Drug Plans with dental coverage

With a Blue Shield Medicare Advantage Prescription Drug Plan, you may receive dental coverage that Original Medicare doesn’t include at no additional premium cost to you. This includes services like routine exams, cleanings, X-rays, fillings, crowns, dentures, and more. For an additional cost you may be able to add even more dental coverage, like implant coverage, with one of our optional supplemental dental HMO or PPO plans.

Optional Supplemental Dental plans for MAPD

Blue Shield of California offers two supplemental dental plans designed to enhance the dental coverage included in the plans above. Depending on the Medicare Advantage Prescription Drug plan you choose, you may choose between an optional supplemental dental HMO¹ plan or an optional supplemental dental PPO² plan. Both options provide a broad range of dental benefits, including many diagnostic and preventive services at no additional cost to you.

HMO plan buy-up

When you choose the HMO plan buy-up dental options it includes:

$16.00 monthly plan premium and enjoy fixed copayments for all covered services
Many routine and diagnostic care services like teeth cleanings and X-rays are available at little to or no cost to you
Specialty care services are available with a referral from your primary dental care provider³
47 additional covered services and lower average copays, compared to the dental benefit that most members have embedded in their Blue Shield MA-PD plan
Choose a participating dentist from our large network that includes 1,000 additional dentists including specialists compared to the embedded dental benefit that is included in many Blue Shield MA-PD plans
No deductibles
No annual maximums
No waiting period
No claim forms

PPO plan buy-up

When you choose the PPO plan buy-up dental option, it includes:

$49.00 monthly plan premium
A broad range of dental benefits, including 100% coverage for most diagnostic and preventive services when received from a participating dentist.
You’re free to see any dentist, but you’ll typically pay less when you choose to receive services from a participating dentist.
Access to 48,136 dentists in California including specialists. If your dentist isn’t in our network, you can still continue seeing them or any out-of-network dentists and receive coverage³
Specialist care available with no referral needed from your dentist³
Includes additional covered services such as dental implants not covered on the embedded dental benefits in many of the Blue Shield Medicare Advantage Perscription Drug plans
A $50 calendar-year deductible applies to services beyond diagnostic and preventive care

You pay the copayment plus the cost of precious or semi-precious metals. Porcelain on molar crowns is not a covered benefit.
