SISC PPO Plan
About the SISC PPO plan
The SISC PPO plan is a self-funded plan administered by Blue Shield of California. With the PPO plan, you can receive care from any of the physicians and hospitals within the plan’s network, as well as outside of the network for covered services.
If maintaining a relationship with your current doctor is important to you, the SISC PPO plan lets you continue seeing your current doctor for most covered services, even if your doctor isn’t part of the plan’s provider network. Keep in mind that if your physician is not part of the plan’s network, you will have to pay more for each visit.
If you are enrolled in a SISC PPO plan that is eligible for a Health Savings Account (HSA), your prescription benefits are administered through Blue Shield of California. To learn about prescription drug coverage, visit Pharmacy Benefits.
If you are enrolled in a SISC PPO plan that is not HSA eligible, your prescription benefits are administered through Navitus Health Solutions.
The BlueCard® Program gives PPO participants access to care across the United States, and urgent care and emergency care around the world. You are not required to use a BlueCard provider; however, it’s in your best interest to use a BlueCard provider to keep your costs down. You can locate a BlueCard provider at any time by calling (800) 810-BLUE or by going to Find a Provider.
