Summary of Benefits and Coverage

The Summary of Benefits and Coverage (SBC) rule is a provision of the Affordable Care Act (ACA). It requires that all carriers, employers and self-insured health plans provide individuals with a uniform summary of their benefits and coverage. The intent of the rule is to provide consumers and customers with an easy way to understand their coverage.

You can go to the Summary of Benefits and Coverage Forms section to download the forms.

Please note, while the development and distribution of SBCs is a new federal requirement these forms do not replace benefit summaries. Please continue using benefit summaries for complete plan details.
