SilverSneakers is a comprehensive program that can improve balance, energy, and overall well-being and social connections. Designed for all levels and abilities, SilverSneakers provides convenient access to our nationwide fitness network, a variety of programming options and activities beyond the gym that incorporate physical well-being and social interaction.
For asthma, chronic obstructive pulmonary disease, congestive heart failure, and diabetes
Care Management Programs — These programs provide you with education to better manage your condition(s). Our nurse works with you and your doctor to help you stay healthy and to avoid the hospital. Your nurse gives you support and education. They also help coordinate your care and to get all your preventive health screenings. You can call to speak with our team at (855) 905-3825 [TTY: 711], 8 a.m. to 8 p.m., seven days per week, and ask any questions.
Care Management has the three programs available for our members. Complex Care Management assists members with chronic condition. The Disease Management Programs help members with the following conditions: asthma, chronic lung disease, congestive heart failure, and diabetes. The Transitions of Care Program helps members after they leave the hospital to make sure that the member has scheduled a visit with their healthcare provider and have all of their healthcare needs (i.e. medical equipment, medication) upon leaving the hospital.

Reaching and keeping good health is a lifelong effort. We offer several online tools that can help you live healthier and manage chronic conditions.

Our health education brochures provide you with information on how to stay healthy and manage your conditions. Brochures are available on the following topics: alcohol and substance use disorders, asthma, alternative medicine, diabetes, family planning, pregnancy, breastfeeding and parenting, high blood pressure, nutrition and weight management, immunizations, smoking cessation, and many more.
Review preventive health guidelines and recommended immunization schedules for Medicare members. These guidelines are based on the recommendations of the U.S. Preventive Services Task Force, the Advisory Committee on Immunization Practices, and the Centers for Disease Control and Prevention.
You can watch videos on health topics like cardiovascular, respiratory heath, diabetes, cancer, wellness, general health and more available in several languages. Videos are provided by Krames on Demand.
