Care Navigator

With a Cal MediConnect plan, you are assigned a Care Navigator to coordinate all your providers and services. They make sure you get the care you need. Your Care Navigator is available to answer your questions. They can refer you to community resources too.

Care team

No more doctor disconnects! With a Cal MediConnect Plan, we help you put together the best team for your individual needs.

Your care team includes:

  • You
  • Your caregiver
  • Your Care Navigator
  • All of your providers (doctors, nurses, counselors, and other health professionals)
 
With help from your care team and Care Navigator, you will be able to direct your own care.

Follow a care plan customized for you.

Your care team works together to coordinate the services you need. For example, your doctors will review all medicines you take, to reduce the risk of side effects.If you have an emergency room visit or hospital stay, your Care Navigator will have a care team meeting. They will make sure you have everything you need to recover safely.

 
Get the best care for your individual needs​
Your care team will work with you to come up with an individualized care plan (ICP). Your ICP tells you and your doctors the services you need and how to get them. This includes medical, mental health, and Long-Term Services and Supports (LTSS). We make sure your ICP stays current. It’s updated at least once a year.
 
Your care plan also includes:
  • Your health goals
  • A timeline for when to get services
