The Cal MediConnect Program was developed by the California Department of Health Care Services, in conjunction with the Centers for Medicare & Medicaid Services, to provide an innovative way for those who qualify for both Medicare and Medi-Cal to access their health care.

Under Blue Shield Promise Cal MediConnect Plan, a single health plan oversees all of your medical, long-term care, mental health, substance use care, and social services needs. As a result, both members and providers benefit from the program's smoother, simpler process. The California Department of Health Care Services and the Centers for Medicare developed the MediConnect plan.

Healthy, independent, empowered lives

Blue Shield Promise Cal MediConnect Plan gives members a voice in their health care, and the ability to self-manage their conditions. We strive to assist our Cal MediConnect members in leading healthy, independent lives by covering unlimited transportation to medically necessary and covered services, providing free Personal Emergency Response System (PERS) devices, home meal delivery and paying for SilverSneakers® gym membership.

Easy access to services and care solutions

Our members have their own personal Care Navigator, a Customer Care representative available via phone seven days a week, 8 a.m. to 8 p.m. Members also have access to a free round-the-clock nurse advice line, health education workshops, a social worker, and more.

Benefits, List of Covered Drugs, pharmacy and provider networks, and/or copayments may change from time to time throughout the year and on January 1 of each year.
