Provider directory information

The information contained in the Blue Shield of California Promise Health Plan directories is gathered from new provider forms and/or contracts. It can also be gathered during office visits, over the telephone, or through the Blue Shield of California Promise Health Plan website.

The information listed in this directory is subject to change. Blue Shield of California Promise Health Plan validates its network information every three months and credentials its participating providers at least every three years. Blue Shield of California Promise Health Plan makes every attempt to validate information contained in the directories. Blue Shield of California Promise Health Plan collects and publishes provider language and language services available through the practice. However, certain information is self-reported and optional such as provider race/ethnicity. This information is available upon request by contacting Member Services in your County.

You may receive services from any of the providers on the list that are affiliated with your PCP’s medical group. For some services, you may need a referral from your PCP. Women may go to any OB/GYN in their PCP's network without a referral. Ask your doctor or call Member Services if you do not know an OB/GYN in your PCP's network.

Interpreting services are available for all providers through Blue Shield Promise Medi-Cal Health Plan at no cost. An advance notice is needed for face-to-face and ASL interpreting services. To request services, contact Member Services in your County. The call is free. For more information, visit the Medi-Cal member site.
