Landmark’s physician-led teams bring medical, behavioral, and social support services to individuals in their homes. Landmark’s mobile team reinforces your in-office care plan with in-home touchpoints.

The Landmark program is available at no additional cost for certain Blue Shield members. Blue Shield determines eligibility.

Frequently asked questions

Does Landmark act as the primary care physician?
No. This program helps support the work you do and provides you with up-to-date information, while being a constant and consistent resource to eligible patients when and where they need care.

How will Landmark communicate with me?
If a patient of yours qualifies for the Landmark program, we will notify you via a mailed letter. After an initial in-home visit, Landmark will contact you with follow-up communication per your preferences, e.g., phone, fax, or mail.

Will Landmark providers change my patient’s care plan?
Landmark works to support your care plan. Based on their findings in the home, providers will update you on changes in your patient’s status, disease progression and medication usage, and proactively partner with you to address issues, except in urgent situations.
