Blue Shield of California Annual Health Assessment Program

You may have received a call as a member of Blue Shield of California to schedule your Annual Wellness Visit.

Blue Shield of California announces a convenient way for you to get a personalized Annual Wellness Visit and complete the Annual Health Assessment. You can even do this in the comfort of your own home.

We have teamed up with our health care affiliate Altais to deliver through your plan at no extra cost. Your visit summary will be available for you and your primary doctor. Your primary doctor can use this information to provide the right care for you.
 

How does it work?

An agent from Altais will call and schedule the in-home visit with you. If you prefer a virtual visit, please let them know. This visit is available to you from now through December 31, 2023.
 

What happens during an Annual Wellness Visit?

Your visit takes up to 60 minutes. The exam will be performed by a local certified nurse practitioner.  

  • Review your medications. Discuss any health concerns you may have.
  • Identify what screenings or recommended tests you may need.  We can help get these set up or completed during your visit.
  • Discuss services that can help you stay healthy and improve wellness.
  • Receive a comprehensive summary that you can share with your primary doctor. This summary can supplement your current care plan.

This visit is not meant to replace your current doctor appointments or care. It is an important piece in providing information to your primary care doctor on potential care plan needs. The assessment may show that you could benefit from other care programs. It may also make sure you are up to date with health screenings.
