How to apply for Extra Help program
To apply for Extra Help, you may:
Visit your local Social Security office.
Apply over the phone, schedule an appointment, or request a paper application.

Once you apply, you will receive an approval letter from Social Security notifying you whether you qualify to receive Extra Help. You will be required to enroll in a Medicare Prescription drug plan.
NOTE: If your application is approved, you are required to enroll in a Medicare prescription drug plan. If you do not select a plan, the Centers for Medicare & Medicaid Services will select a plan for you. The sooner you join a plan, the sooner you begin receiving benefits. If Social Security believes you do not qualify for Extra Help, you will receive a Pre-decisional notice. After receiving this notice, you will have 10 days to visit your local Social Security office or contact them at (800) 772-1213 to provide additional information.
