As a Blue Shield of California member, you may have access to in-home care services provided by ConcertoCare®.
The ConcertoCare team brings care services to your home when you need them. They work with your primary care doctor and other providers to coordinate your care needs. Their goal is to help you stay healthy with fewer hospital visits.

ConcertoCare is a healthcare company that helps take care of you when you're sick or need medical help. They work with your regular doctor to offer medical support and care you need. ConcertoCare can visit you at your home or provide care and support by phone or video.
The ConcertoCare approach
ConcertoCare does not replace your primary care doctor or care team. They offer added support in coordination with your care team. They are physicians, nurses, pharmacists, and health coaches who work alongside your primary care provider and/or specialty providers to deliver complete care. They have a provider on-call and available 24/7.
ConcertoCare provides a care team designed for you. They support you, your family, and those who care for you.

This is a program for certain eligible Blue Shield of California members. Eligible members will receive a letter inviting them to participate. Eligibility is determined based on health status and needs.
For Blue Shield Commercial plans: ConcertoCare is independent of Blue Shield of California and is contracted by Blue Shield to provide in-home and virtual care services. These services are not covered benefits of Blue Shield health plans and none of the terms or conditions of Blue Shield health plans apply. Members who are not satisfied with products or services received from ConcertoCare may use Blue Shield's grievance process described in the Grievance Process section of the Evidence of Coverage or Certificate of Insurance/Policy. Blue Shield reserves the right to terminate this program at any time without notice.
For Blue Shield Medicare plans: The products and services described are neither offered nor guaranteed under Blue Shield of California’s contract with the Medicare program. In addition, they are not subject to the Medicare appeals process. Any disputes regarding these products and services may be subject to the Blue Shield grievance process. Should a problem arise with any value-added item or service, please call Customer Service at the number on the back of your Blue Shield ID card.
