Lifestyle Medicine Programs

At Blue Shield, we know that your relationship with your patients strongly influences their knowledge and choices regarding their health. That’s why we want you to know about the lifestyle medicine programs we offer.

Lifestyle medicine involves the use of evidence-based lifestyle approaches, such as a predominantly whole food, plant-based diet, exercise, sleep, stress management, and support systems to prevent, treat and oftentimes reverse lifestyle-related chronic disease.

We urge you to partner with us in telling your Blue Shield patients about the lifestyle medicine programs listed below so they can take advantage of all opportunities available to them to live their healthiest lifestyle.

Diabetes Prevention Program

More than 86 million Americans have prediabetes, or elevated blood sugar levels, and most don’t even know it. Through the Diabetes Prevention Program, patients who qualify for the program learn ways to eat healthier, increase their activity, and manage the challenges that come with lifestyle changes to help them prevent type 2 diabetes.

One-minute quiz helps patients find out if they qualify for the program

As part of their Wellvolution benefits, Blue Shield members can take a short online quiz to find out what their risk level may be for type 2 diabetes. If they qualify, they follow the process available through the program to select a provider who can work with them to focus on their specific needs and enroll immediately in the program.

Provider resources for the Diabetes Prevention Program


Doctor Dean Ornish’s Program for Reversing Heart Disease®

The Program for Reversing Heart Disease is an intensive program that is available to members by physician referral only. This program helps patients make improvements to four key areas of their lives: nutrition, stress management, fitness and social support.
  • Nine-week outpatient series, with two four-hour sessions a week
  • Small classes allow personal support in choices, habits, and well-being
