Outpatient Facility, Psychiatric Hospital, and Residential Treatment Center 

Upon submission of a completed application packet, Blue Shield will begin the review process. If the application is approved to move forward, you will be contacted to begin the contracting process.

Facility application

Applications are to be dated no more than thirty days from submission and either emailed or faxed to:

The standard turnaround time for processing completed applications is between 45 and 60 days.

For NEW individual providers/provider groups

MD, NP, Psychologist, LMFT, LCSW, and LPCC

Upon submission of a completed application packet, signed contract, and rates as outlined, the credentialing process will begin. 

Individual provider/Provider group application

Applications are to be dated no more than thirty days from submission and either emailed or faxed to:

Processing and participation

The application and corresponding documents are reviewed and formally approved or denied by Blue Shield’s Credentialing Committee. Upon approval, the Contracting Department will send a welcome letter with the contract effective date.

Services rendered to Blue Shield members prior to this effective date will process as out-of-network.
