For all Blue Shield direct contracted providers
Upon submission of a completed application packet, Blue Shield will begin the review process. If the application is approved to move forward, you will be contacted to begin the credentialing and contracting process.

Complete both the Letter of Intent and Record Application Form and submit via email to:

Applications are to be dated no more than thirty days from submission
The standard turnaround time for application review is between 30 and 60 days.
If approved to proceed, the below documents are required to begin contracting. Please assure these are ready at time of application submission.
Individuals
- Individuals must have active license and Type 1 NPI
- If using EIN a W-9 or Pre-printed Tax Document from IRS (if new provider)
- Active Credentialing for contracting
Group
- Type 2 NPI
- If using EIN a W-9 or Pre-printed Tax Document from IRS (if new provider)
- If incorporated – Articles of Incorporation from CA Secretary of State
- If using a Fictitious Name a Fictitious Name Permit or Fictitious Name Statement may be required.

Processing and participation
Note: If you submitted a contract to Blue Shield prior to 4/28/2025, there is no need to submit an application packet. If you have questions, please contact BSCProviderNetworks@blueshieldca.com.
For direct contract providers, receipt of credentialing approval does not provide you with participating status in network. Blue Shield will assign the effective date for your participating status based upon approval from the Credentialing Committee and the Contracting Department. Services rendered to Blue Shield members prior to this effective date will process as out-of-network.
A copy of the executed Independent Provider Agreement including the assigned effective date will be returned to you after processing.
