Community Supports are extra services that Blue Shield of California Promise Health Plan offers to eligible Medi-Cal members. They are not benefits. These services provide support to members beyond Long Term Care Support Services (LTSS). They enhance and support members’ care. Services will vary based on a member’s needs and on Blue Shield Promise rules. Prior approval is required.
Blue Shield Promise offers Community Supports to qualified members in Los Angeles and San Diego counties:

Members receive help finding, coordinating, securing, or funding one-time services and modifications needed to set up a basic household. |

Members get help maintaining safe and stable tenancy after housing is secured. |

Nutritional support for members, especially those who live with chronic or serious conditions for which meeting nutritional goals is critical to the member getting healthy and staying healthy. |

Help members who have an imminent need for nursing-facility-level care continue living in the community, avoiding institutionalization whenever possible. ALF includes both Residential Care Facilities for the Elderly (RCFEs) and Adult Residential Care Facilities (ARFs). |

Members receive help with Activities of Daily Living (ADLs) such as bathing, dressing, toileting, ambulation, or feeding; Instrumental Activities of Daily Living (IADLs) like meal preparation, grocery shopping, and money management; and personal care services like bladder and bowel care, bathing, and grooming; for Members who could not otherwise remain in their homes without these supports. |

Short-term residential care for those who need to recover from an injury or illness (including a behavioral health condition) and have medical needs significant enough to result in emergency department (ED) visits, hospital admissions, or other institutional care. Recuperative Care allows individuals to access primary care, behavioral health services, case management, and other supportive social services, such as transportation, food, and housing, while recovering. |

Short-term, non-medical relief from caregiving duties for those who take care of Medi-Cal members who require intermittent temporary supervision. |

A place for members with high medical or behavioral needs who are experiencing or at risk of homelessness to continue recovering immediately after exiting an inpatient medical/psychiatric/substance use disorder hospital or residential facility, including recuperative care. |

Safe, supportive places for individuals to become sober after being found publicly intoxicated due to alcohol or drugs and help for individuals experiencing homelessness or unstable living situations who would otherwise be sent to the emergency department or jail. |
| Transitional rent (effective January 1, 2026) | Transitional rent supports Medi-Cal members experiencing or at risk of homelessness with up to six months of rental assistance for use in interim and permanent settings. Members must have certain clinical risk factors and have either recently undergone a critical life transition (such as exiting an institutional or carceral setting or foster care) or meet other specified eligibility criteria. |
