As an HMO member, you are assigned a primary care physician (PCP) during enrollment. Your PCP provides care and coordinates referrals within your medical group.

Trio HMO plan

Plan highlights:
- Access to Trio HMO network providers only
- You must live in the Trio HMO service area
- Each member must select a primary care physician (PCP)

- Enhanced assistance from Shield Concierge
PPO plan

- Deductible (family): $300
- Primary care office visit: $20

- Freedom to visit any doctor—in or out of our PPO network
- Self-refer to specialists—no PCP referral required
- $0 copay for virtual care (medical and mental health)
- Costs include coinsurance after meeting the deductible
- Coverage for medical care across the United States
