Blue Shield Medicare (PPO) plan
2027 out-of-pocket costs (network):
- Deductible: None
- Primary care: $5 copay/visit
- Specialist: $15 copay/visit
- Emergency room: $65 copay/visit
- Generic Drugs: $5/30-day supply
