Consent to use of Self-Service Portal website

This website (the “Self-Service Portal” or “Site”) is owned and operated by Blue Shield of California, and its affiliates, including Blue Shield of California Life & Health Insurance Company (collectively, “Blue Shield”). Please read the following terms and conditions ("Terms of Use") carefully and completely before using the Site.

Access to and use of the Self-Service Portal is intended solely for authorized users of Blue Shield customers. By accessing and using the Site and the links accessible through the Site, you expressly represent that you are an authorized user of a Blue Shield customer and that you agree to be bound by the Terms of Use and any other terms and conditions set out throughout the Site, as well as any terms and conditions applicable under the terms of your organization’s contractual arrangements with Blue Shield. The information, materials and resources contained on and accessible through the Site are made available to you by Blue Shield and its suppliers and vendors, subject to your agreement to the terms and conditions listed herein. Please note that use of some areas of the Site may be subject to additional terms and conditions.

Any activity on this Site is subject to monitoring by Blue Shield. By using this Site, you are consenting to such monitoring and agree that Blue Shield may use the results of such monitoring without limitation.

If you do not agree with these Terms of Use, please do not use the Site.

It is a condition of your use of this Site that you do not restrict or inhibit any other authorized user from using this Site.
 

Attestation of authorized use

By accessing the Self-Service Portal, I hereby agree and acknowledge that:

1. I agree to be bound by the Terms of Use.
2. I am an employee, agent, or representative of an organization that is authorized by Blue Shield of California to access the Self-Service Portal (“Authorized Organization”).
3. I have been authorized by the Authorized Organization to access the Self-Service Portal for purposes related to the administration of the Authorized Organization’s benefit plans and programs.
4. I understand that all of the information accessible through the Self-Service Portal is considered confidential information and that the information may include protected health information (PHI) as defined by the Health Insurance Portability and Accountability Act of 1996, as amended (HIPAA).
5. I agree that I will access, use, and disclose the information in the Self-Service Portal only for purposes of the administration of the benefit plans and programs sponsored by the Authorized Organization and in a manner consistent with any applicable Authorized Organization or Blue Shield policies. I understand that my access rights will extend only to data for Authorized Organization members.
6. I understand that I have the following obligations for purposes of my use of and access to the Self-Service Portal:
   a.  To limit my access and use of the Self-Service Portal to permissible purposes in my role as an Authorized Organization employee, agent, or representative involved in the administration of the Authorized Organization’s benefit plans and programs;
   b. To retain all information in the Self-Service Portal in strict confidence and not to access, use, or disclose the information in a manner that would violate applicable Authorized Organization or Blue Shield policies regarding the confidentiality, privacy, or security of the information; and
   c. To comply with all applicable legal requirements related to my access and use of the Self-Service Portal.
7. I will immediately report to an appropriate Authorized Organization or Blue Shield representative any incident involving the Self-Service Portal or information contained in the Self-Service Portal that I reasonably believe may violate applicable Authorized Organization or Blue Shield policies, including the Terms of Use. I understand that this reporting obligation extends to both intentional and unintentional violations and to incidents involving myself or others.
