2027 out-of-pocket costs (network):
- Primary care: $10 copay/visit
- Specialist: $10 copay/visit
- Urgent care: $0 copay/visit
- Emergency room: $0 copay/visit
2027 network pharmacies 30-day supply (preferred cost-sharing):
- Generic Drugs: $5 copay
- Preferred Brand Drugs: $20 copay
- Non-Preferred Drugs: $35 copay
- Specialty Tier Drugs: $50 copay
