Do you qualify?

Most people with full Medicare and Medi-Cal benefits in Los Angeles and San Diego counties are eligible for Blue Shield Promise Cal MediConnect Plan.

 

Are you eligible for Cal MediConnect?

To check eligibility, call us toll-free at (844) 758-0387 [TTY: 711], 8 a.m. – 8 p.m., seven days a week.

  • Live in Los Angeles or San Diego counties AND
  • Qualify for both Medicare and Medi-Cal AND
  • Are 21 or older at time of enrollment

You may not qualify if you are:

  • Covered under an employee benefit plan 
  • Have other private or public health insurance
  • Receive services through California’s regional centers or state developmental centers or intermediate care facilities for the developmentally disabled
  • Have ESRD (End Stage Renal Disease) 

If you live in a nursing home, are enrolled in the Multipurpose Senior Services Program, or have In-Home Supportive Services, you must continue to meet your share of cost.

With a Blue Shield Promise Cal MediConnect Plan, you get Medicare and Med-Cal benefits and additional benefits valued at over $5,400. The additional benefits and estimated value amounts are hearing aid benefits -$2,000, eye exams and up to $500 for eyeglasses (frames and lenses) or contact lenses every two years (benefit period), $0 for SilverSneakers® fitness and wellness programs- $600, unlimited transportation to plan-approved locations - $1,200, You are entitled to a quarterly allowance of $185 for OTC -$740, $0 PERS is a medical alert monitoring system $240. You pay $0 for each covered home meal delivery - $380. Blue Shield of California Promise Health Plan’s actuarial estimate of these benefits are assuming maximum coverage over course of 2021 for Blue Shield Promise Cal MediConnect Plan in Los Angeles and San Diego counties.
