HMO plan
With the Blue Shield HMO plan, you:
  • Only pay a fixed copayment for most services like doctor visits, urgent care, and emergency care.
  • Have access to one of the largest provider networks in California, with more than 10,000 primary care physicians (PCPs), 270 hospitals, and 34,000 specialists.
  • Choose a PCP who will coordinate your care and refer you to specialists and hospitals within their medical group/Independent Practice Association (IPA).

Note: When you enroll in the Blue Shield HMO plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/IPA.
Trio ACO/HMO plan
Our Trio ACO/HMO plan:
  • Has no deductible, just fixed copays.
  • Is typically more affordable than other plans.
  • Features a select network of local providers who closely coordinate your care.
  • Offers a concierge service to help you with everything from benefit questions to scheduling doctor’s appointments.
Note: When you enroll in the Trio ACO/HMO plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/IPA.
PPO plan
The PPO plan allows you to:
  • See any doctor you choose for most services.
  • Self-refer to specialists.
  • Usually pay less for services that are provided by PPO network providers.
  • Pay a copayment or coinsurance for most services after the plan’s deductible has been met.
High Deductible Health Plan (HDHP)
With the High Deductible Health Plan, you can:
  • See any doctor you choose for most services.
  • Self-refer to specialists.
  • Usually pay less for services that are provided by PPO network providers.
The plan is paired with a federal tax-free health savings account (HSA)* to help you save money. The HSA works a lot like a savings account. You contribute pre-tax dollars and use them to pay for your deductible and qualified out-of-pocket medical expenses.
Tandem ACO/PPO plan
Our Tandem ACO/PPO plan:
  • Offers the flexibility of a PPO plan but is typically more affordable due to a select network of providers.
  • Includes over 65% of doctors and 90% of hospitals from our broad PPO network.
  • Offers concierge support services to help answer your questions about benefits, claims, and more.

Current Blue Shield member

If you are a current Blue Shield member, complete the following form to enroll:
Continuing Group Coverage after Federal COBRA Cal COBRA Election Form
After exhausting 18 months of Federal COBRA benefits, a beneficiary may be eligible to an 18-month extension through Cal COBRA. Beneficiary must contact Cal COBRA at (800) 228-9476 to request the extension and ensure they meet the requirements. If approved, the beneficiary will submit this form to formally accept the extension.

Not a Blue Shield member

If you are not a Blue Shield member, complete the following form to enroll:
Cal COBRA Take-Over Form
New groups should use this form when changing carriers to Blue Shield for Cal COBRA members covered under a previous carrier. Employers are responsible for notifying their Cal COBRA members of the transition to a new carrier, and Cal COBRA members are required to fill out the form and submit to the Cal COBRA team within 30 days of transition.

Prescription drug benefits are an important part of your Blue Shield health plan.
Understanding your pharmacy benefits can help you save money. You may save if your medication is a preferred prescription drug. Also, if you take a stabilized doses of covered medications for chronic conditions such as diabetes, you can have a 90-day supply delivered through our mail service pharmacy. Shipping is free, and you may save on your copay.

With Care Management from Shield Support, you’ve got a team of nurses, health coaches, and other specialists by your side.
Shield Advocate
Our Shield Advocate program offers short- and long-term support to optimize your benefits, health, and quality of life. An outreach specialist will connect with you to understand your needs, discuss how we can best support you, and connect you with a care manager. You can be certain that your medical history and information will be kept confidential. Our experienced care teams are here to help.
