Providence Access+ HMO plan
With the Access+ HMO® plan, you:
When you enroll in the Access+ HMO plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/IPA.
- Have no deductible – just fixed copayments for most services like doctor visits, urgent care, and emergency care
- Have access to one of the largest provider networks in California, with more than 13,000 primary care physicians (PCPs), 50,000 specialists, and 360 hospitals
- Choose a PCP who will coordinate your care and refer you to specialists and hospitals within their medical group/Independent Practice Association (IPA)
When you enroll in the Access+ HMO plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/IPA.
Providence OptionPLUS HMO plan
Blue Shield and Providence Health & Services Caregivers developed this plan for Providence Southern California employees.
Important: The difference between this plan and Blue Shield's Access+ HMO plan is the provider network. This plan offers a smaller, specially selected network.
With the Providence OptionPLUS HMO plan, you:
When you enroll in the Providence OptionPLUS HMO plan for the first time, you must choose a PCP for yourself and your covered dependents. Each member of your family can choose a different physician and medical group/IPA.

- Pay a lower employee contribution
- Have access to a smaller, specially selected network of Providence-affiliated medical groups and primary care physicians (PCPs) and specialists than is available with the Access+ HMO plan
- Choose a PCP who will coordinate your care and refer you to specialists and hospitals within their medical group/Independent Practice Association (IPA)

Access+ HMO providers
Use our Find a Doctor tool to see if your doctor is in the Access+ HMO Network and to find specialists, urgent care centers, and other providers.
To search for a primary care physician (PCP) in the Find a Doctor tool, select Primary Care Physician, and enter your location. You can then search for a PCP by selecting Doctor Type, Doctor Name, or Medical Group.
When you enroll in the Access+ HMO plan for the first time, you may need your selected PCP’s ID number. To find this number, once you locate your PCP's name in the Find a Doctor tool, click on the doctor’s name and then select View details under “Primary Care Physician ID.”
