As your health plan, we want to help keep you and your family healthy. Our Health Education and Health & Wellness Programs are here to help you set up a plan to:

  • Eat healthier
  • Be more active
  • Maintain your weight
  • Quit smoking, if you smoke
  • Manage chronic conditions, like diabetes, asthma and high blood pressure
Health & Wellness Program

We know reaching and keeping good health is a lifelong effort. To help you, we offer the Health & Wellness portal (website). The portal has many tools that can help you live healthier and manage chronic conditions. You'll have access to:

  • Daily calorie plans and recipes
  • Exercise plans and demonstrations
  • Health information (brochures and videos)
  • Health workshops
  • Health coaching by phone or e-mail
  • A Registered Dietitian (nutrition expert) and a Personal Trainer by e-mail
  • Activities that focus on helping you with back problems, control high blood pressure, improve your mood and much more
As part of our 2019 Healthy Rewards Program, from March 1, 2019 to November 30, 2019, you will earn points toward an incentive prize when you complete certain activities on the portal.

Once you create an account, answer the questions on the Personal Health Assessment (PHA). Once you complete the PHA, you will have access to the portal and can start earning points. New program that helps prevent Type 2 Diabetes

You now have access to a new program that helps prevent type 2 diabetes. This program is called the Diabetes Prevention Program (DPP). We have partnered with Solera Health to offer you this service at no cost to you. This program will help you learn about ways you can be more active, eat better and lose weight.

You will learn about the importance of making small changes to your daily routine. For example:

  • increasing your physical activity
  • eating better
  • losing weight

These changes can have a big impact on your health. The goal is to lose 5-7% of your body weight, which can reduce your risk for type 2 diabetes. We know it is hard to make lifestyle changes and stick with them. If you qualify you will meet with a health coach and small group for support and encouragement.

The program lasts 12 months. You will meet at a location close to you for:

  • 16 weekly sessions over the first 6 months
  • Monthly session during the last 6 months

Help for Back Pain

Have you ever said, “ouch, my aching back”? Then you are not alone. According to the National Institute of Arthritis and Musculoskeletal and Skin Disease, back pain affects 8 out of 10 people at some point in their lives. Some people experience acute pain that normally lasts less than 6 weeks, while others experience chronic pain, which can last longer than 3 months.

Our online Health & Wellness portal has many resources to help you with your back problem. You’ll find healthy back workshop, audio and video presentations, and reading materials. You can also view demos on exercises for strengthening your back. You can even e-mail questions to a Fitness Trainer.
