Just follow the steps below and remember to include the following forms:

Step 1 Producer Agreement
blank
  • Read and complete the attached Producer Agreement, sign, date, and return the last page of the Producer Agreement. 
Step 2 California Life Agent License

  • Include a copy of your California Life-Only and Accident & Health agent licenses and Errors & Omissions Insurance Certificate.
  • The expiration date must be more than 30 days from the date your paperwork is received by Blue Shield.
Note: The agent or company applying must be noted on the California Life Agent license, and there should be no restrictions on the license.

Step 3 Direct Deposit

  • Complete and return the attached Direct Deposit Authorization Form. Blue Shield of California is now paying commissions exclusively through Electronic Funds Transfer (EFT), and your appointment cannot be completed without this form.
Step 4 Errors & Omissions (E&O) Insurance

Include a copy of your E&O insurance certificate, and note that:

  • The certificate must show a minimum specific and aggregate amount of $1,000,000 each.
  • The insurance policy must be valid at the time the paperwork is submitted. Also, the agent or company applying must be noted on the certificate.
Note: If you are applying for E&O insurance through MGA Insurance Services on behalf of Blue Shield of California, please include a copy of your MGA application. For more information, go to www.bcs-eo.com/cabs.

If you are submitting your Blue Shield Producer Agreement and other documents along with a client's application for healthcare coverage, please note that California law requires that applicants be presented with a copy of the Uniform Health Plan Benefits and Coverage Matrix, disclosure form, and rate quote at the time of sale. Include your Tax ID number on any IFP, Medicare Supplement, or Master Group applications submitted.
