Medical Plans/Networks

Access + HMO You choose a Personal Physician, who coordinates your care, including referrals to specialists. These specialists are within your physician's medical group. If your health plan offers the Access + Specialist program and your Personal Physician participates in the program, you may see a specialist in your physician's medical group without a referral. These specialists are designated with an "A+." You and all covered family members must live or work in the Access+ HMO service area to enroll in this plan.
Access + HMO Save Net You choose a Personal Physician, who coordinates your care, including referrals to specialists. The Access+HMO SaveNet network and service area are by design smaller in scope and reach than the Access+ HMO network. While the SaveNet network of physicians is chosen from Blue Shield of California's entire Access+ HMO physician network, it offers access to all Access+ HMO network hospitals. If your Personal Physician participates in the Access + Specialist program, you may see a specialist in your Personal Physician's medical group without a referral. These specialists are designated with an "A+."
Trio ACO HMO The TRIO ACO HMO health plan requires you to select a Personal Physician who coordinates your care, in collaboration with specialists and selected local hospitals. The TRIO ACO HMO network and service area are by design made up of a select network of physicians from Blue Shield of California's entire Access+ HMO physician network and offers admission to all Access+ HMO network hospitals. These physicians participate in Accountable Care Organization (ACO) programs with local specialists and select hospital facilities designed to help coordinate your care and improve your overall healthcare experience.
The TRIO ACO HMO plan is available only to members who live or work in the plan's service area defined by select ZIP codes. To take advantage of the TRIO ACO HMO and its network of providers, all family members must be enrolled in the same plan. If your Personal Physician participates in the Access+ Specialist program, you may see specialists in your Personal Physician's medical group without a referral. These specialists are designated with an "A+".
HMO Network 1 for SHOP You choose a Personal Physician, who coordinates your care, including referrals to specialists. The HMO Network 1 for SHOP physician network is selected from Blue Shield of California's entire Access+ physician network, and it offers access to all Access+ HMO network hospitals. The service area includes select ZIP codes, and the plan is available only to members who live or work in the plan's service area. To take advantage of the HMO Network 1 for SHOP and its network of providers, all family members must be enrolled in the same plan. If your Personal Physician participates in the Access + Specialist program, you may see a specialist in your Personal Physician's medical group without a referral. These specialists are designated with an "A+."
HMO Network 2 for SHOP You choose a Personal Physician, who coordinates your care, including referrals to specialists. These specialists are within your physician's medical group. If your health plan offers the Access + Specialist program and your Personal Physician participates in the program, you may see a specialist in your physician's medical group without a referral. These specialists are designated with an "A+." You and all covered family members must live or work in the HMO Network 2 for SHOP service area to enroll in this plan.
Local Access + HMO You choose a Personal Physician, who coordinates your care, including referrals to specialists. The Local Access+ physician network is selected from Blue Shield of California's entire Access+ physician network, and it offers access to all Access+ HMO network hospitals. The service area includes select ZIP codes, and the plan is available only to members who live or work in the plan's service area. To take advantage of the Local Access+ and its network of providers, all family members must be enrolled in the same plan. If your Personal Physician participates in the Access + Specialist program, you may see a specialist in your Personal Physician's medical group without a referral. These specialists are designated with an "A+."
2016 Individual and Family PPO Plan (including Covered California) Blue Shield of California's Individual and Family Plan (IFP) PPO Network allows members to access any of our in-network physicians and hospitals at in-network benefit levels. No Referrals are necessary - members can visit any physician or specialist in the IFP PPO Network for lower out-of-pocket costs for covered services. Members can also access out-of-network physicians and hospitals for higher out-of-pocket costs for covered services.
Providence OptionPLUS HMO plan You choose a Personal Physician, who coordinates your care, including referrals to specialists. The Providence OptionPLUS HMO physician network includes Providence-affiliated medical groups, Personal Physicians, and specialists and all Providence Southern California acute care hospitals. The service area includes select ZIP codes, and the plan is available only to members who live in the plan's service area. To take advantage of the Providence OptionPLUS HMO plan and its network of providers, all family members must be enrolled in the Providence OptionPLUS HMO plan and select a medical group and Personal Physician from the Providence OptionPLUS HMO network. If your Personal Physician participates in the Access + Specialist program, you may see a specialist in your Personal Physician's medical group without a referral. These specialists are designated with an "A+."
Blue Shield of California PPO No referrals are needed - visit any specialist in the plan's network. Your out-of-pocket costs will be lowest when you use preferred providers for covered services.

Tandem PPO Network Blue Shield's Tandem PPO plan features a select network of providers from our full PPO network, and offers the same benefits as the full PPO at a lower cost. Like other PPO plans, Tandem offers members the flexibility to see any doctor or specialist in the Tandem network without a referral. The Tandem PPO plan also features care coordination through a Primary Care Physician (PCP). After enrolling in a Tandem PPO plan, members will be matched with a PCP. As a member, your PCP will work with you every step of the way to make sure you are receiving the right care.
SG Exclusive PPO for SHOP Blue Shield of California's Group PPO Narrow Network allows members to access any of our in-network physicians and hospitals at in-network benefit levels. No Referrals are necessary - members can visit any physician or specialist in the Grouop PPO Narrow Network for lower out-of-pocket costs for covered services. Members can also access out-of-network physicians and hospitals for higher out-of-pocket costs for covered services.
UC Care Plan UC Select Providers Choose from two tiers of providers. When you receive care from a UC Select provider, you will have the lowest cost for most covered services. When you receive care from a Blue Shield Preferred provider, you have greater flexibility and choice at a coinsurance level. UC Select: Includes most UC providers, medical centers/hospitals, as well as select Blue Shield PPO network providers and hospitals. Blue Shield Preferred: Includes most Blue Shield PPO network providers and hospitals.

If you need a printed Medicare provider directory, please contact Medicare Member Services at 800-776-4466 or email MemberSvcs@blueshieldca.com. With a Medicare Advantage HMO plan, you have a Personal Physician who will be responsible for providing you with basic and routine medical care. You will generally need a referral from your Personal Physician to obtain care from Specialists associated with the same physician group as that of your Personal Physician.

Vision Providers

Discount Vision Providers (No plan needed) Ophthalmologist, Optician, and Optometrist providers that offer discounted rates. These providers are not covered by Blue Shield vision insurance (members will need to pay out-of-pocket).
Eye Exam Only Plan (Large Group only) Optometrist and Ophthalmologist providers that provide eye exams only that are covered by Blue Shield.
Vision Plans (Individual and Family or Group plans) Ophthalmologist, Optician, Optometrist, and Wholesale/Warehouse providers contracted under Routine Care - Vision Plan Members Program.

Dental Providers

Dental HMO Dental HMO. Choose a primary care dentist.
Dental PPO Dental PPO. No primary dentist needed.

Medical Plans for Government and Public Employees

CalPERS Access + HMO and Access+ HMO Supplement to Original Medicare Plans Blue Shield HMO plan for CalPERS (California Public Employees' Retirement System) members and CalPERS members who have Medicare coverage. You must live or work in the Plan Service Area to enroll.
CalPERS NetValue HMO Blue Shield HMO plans for CalPERS (California Public Employees' Retirement System) members and CalPERS members who also have Medicare coverage. The same benefits as with the CalPERS Access+ HMO plan, but you choose your PCP from a subset of the CalPERS HMO network of medical groups. You must live and/or work in the Plan Service Area to access Plan benefits at these providers.
CalPERS Access + EPO Blue Shield's Exclusive Provider Organization (EPO) plan delivers comprehensive, flexible health coverage throughout certain counties. As an EPO plan member, you don't need to select a Personal Physician. You must live or work in the Plan Service Area to enroll.
CalPERS Medicare HMO Blue Shield Medicare HMO plan for CalPERS (California Public Employees' Retirement System) members.
