Your evidence of coverage (EOC) or Certificate of insurance (COI)

This is the Blue Shield document that describes your benefits, copayments, exclusions and limitations.

The EOC/COI is your plan document. This booklet will explain in full all the terms of your plan coverage, the benefits, copays or coinsurance, exclusions, limitations and how to access coverage.

An EOC or COI was sent to you when you first joined Blue Shield. If you would like to receive a copy of your EOC or COI:

• Go to the Benefits and coverage plan resources page to view and download your EOC/COI (not available to all members). 
• Contact your Human Resource department for a copy if you have health care through your company.
• Contact Us and request that a copy be mailed to you.

Self-funded health plans
If your Employer self funds their Employee health plan, contact your Human Resources Department to obtain a copy of the Plan Document/Employee Booklet to determine how your Mental Health benefits are administered.
