Using Your PPO Option
When accessing skilled nursing facility care under your PPO options, you will receive the highest level of benefits when you access care from skilled nursing facilities in our preferred provider network.
You will also receive the highest level of benefits when you access care from the home healthcare providers in our network.
Your physician will help you obtain approval for these services and will arrange for your admission to a skilled nursing facility or for the provision of your home healthcare.
