Provider network

Blue Shield's provider networks are a list of doctors and other medical professionals that have contracted with Blue Shield to provide services to our members.

If you use a provider in our network this will help you get the best benefits out of your health plan. It will also keep your out-of-pocket costs lower.

Also, some services are only covered when provided you use our providers. Look at your Evidence of Coverage (EOC) or Certificate of Insurance (COI) for more information.

An EOC or COI was sent to you when you first joined Blue Shield. If you need another copy:

• Go to your Plan Summary page and download a copy or ask to have a copy mailed to you (not available to all members).
• Ask your Human Resource department for a copy if you have health care through your company.
• Call the Member Services number on your Blue Shield ID card and ask to have a copy mailed to you.
