Coronary Artery Disease & Heart Failure Programs
If you've had a heart attack, bypass surgery or angioplasty, you may be eligible for Blue Shield's Coronary Artery Disease (CAD) Program. If you have been diagnosed with heart failure, you may be eligible for Blue Shield's Heart Failure Program. Both programs are designed to:
- Help you learn more about your condition
- Keep your doctor aware of how you are doing
- Keep you healthy and out of the hospital by helping you:
- learn to manage your symptoms
- adhere to medications
- deal with stress
- maintain a healthy lifestyle

Applying to either program is easy. Just fill out a short form and submit it online.
